Talk to Head Eye-Mechanic Nimble, who will confess that Gareth’s eye isn’t finished yet because he’s out of lenses.

Head out of the Thylysium East Gate and kill bandits (brigands/bandits/thieves etc.), picking up the eyes they drop. When you have 10 eyes return to Nimble.
